Use the "action" buttons to navigate through the application
- Next Section - takes you to the next section of the application
- Previous Section - takes you back to the previous section of the application without losing any of the information you have
entered
- Clear Section- clear the information on the current section without losing information from any other section
- Dates should be entered as Day/Month/year (e.g., May 29, 2010 = 29/05/2010)
- The following information is required if reporting an adverse event for drugs:
- Describe event or problem;
- Name (Product Name) ;
- The reporter's personal information is required in order to receive an e-mail acknowledgment of the form's receipt from Acino-Middle East.
